INSURANCE: I am able to accept Aetna insurance. Patients with out-of-network coverage may be eligible for partial or full reimbursement from their insurance companies. Payment in full is expected at time of service. You will be issued a “superbill” which you can submit to your insurer to seek reimbursement.
MISSED APPOINTMENTS: You will be charged full session rate for any appointment that is cancelled with less than 24 hours notice. You are required to have a credit card on file so that it can be charged automatically in accordance with the policy.
